10/15/2023 EnchantaVaughn Approved Behavior: Upon arrival to the home setting, Client was non-compliant for most of the session, refusing to conduct herself appropriately and show leadership skills during the activity. Client is often observed showing frequent anger reactions, in which she exhibited oppositional defiance. It was difficult for Client to stay on task. Behavior: Client appeared very moody today for no reason neglecting her duties in the home and in the session. Client did not take constructive criticism well and proceeded to walk away without permission. Intervention: QMHP-C developed a plan to help Client with her challenges in the home. QMHP-C utilized role playing to demonstrate the calming effects of predicting the thoughts, feelings and actions of self and others prior to the outbreak of a conflict. QMHP-C prompted Client to share anticipated thoughts and feelings and to indicate how this process reduces angry reactions. QMHP-C role played with Client to assist her in bettering her social-emotional-mental skills. QMHP-C completed an activity with Client that allowed her to write the words of the thoughts and emotions that she feels when she thinks of her family and close relationships. Intervention: QMHP-C discussed Client’s responses with her and modeled for her how to approach her family/friends about her feelings, good or bad. QMHP-C educated Client about using good manners in the home and offering alternative methods and suggestions to model appropriate language that Client should use to ask for things or to get someone’s attention. QMHP-C processed Client feelings and offered choices of what to do when faced with trauma and conflict. QMHP-C retrained Client on relaxation skills, which included pace breathing, muscle relaxation, and calming imageries as a way for Client to regain and control her rebellious and hyperactive conduct. Response: Client participation level was under the required expectation when practicing calming techniques, as evidenced by Client flat mood and demeanor. Client looked disordered, indicating that she didn’t know the reason for her negative behaviors. Client displayed a disinterest in the therapeutic activities provided by QMHP-C, due to avoiding discussion and prompts. Client was unable to relax when being instructed by the use of the calming and relaxing techniques. Client was resistant and reluctant in regard to QMHP-C positive empathetic acceptance. Client was inattentive and unfocused throughout the session. Response: Client informed QMHP-C of her behaviors after QMHP-C confronted her again about her behaviors. Mother collaborated with counselor in coming up with consequences for certain behaviors displayed in the home. Mother verbalized that she is trying her best to implement consequences even if the principal is small to teach Client the level of seriousness that comes with having mental issues.
